Random items being thrown after battle

After I'm done with a battle sometimes a item keeps being thrown out from my inventory. I've been lucky with it so far and noticed the item on the ground however I just lost a scroll of water because of it.

Sadly no. I down the modpack which fixed the throwing problem but i don't have any proof
