Did not recieve Pokemaster title + Special Forms key

Good morning, I am currently having an issue where I have completed the pokedex, but only received the ultra crate key. If anyone could help with sorting this out I would greatly appreciate it. Here my IGN and server I play on:

IGN: CRAPYAUL
server: Sapphire

I have also attached a couple screenshots that show my /dex count & the notification when I completed my dex and only received an ultra crate key. I would like to point out I am a bit late in making this post, but I went on vacation the day I completed my dex, which was August 7th. Thank you in advanced for any help!
